What Happens After You Refer?
Submit a Secure Referral
Complete BridgePoint’s short provider referral form with the information you have available. You do not need to provide every detail before submitting a referral.
The provider referral for is designed to take approximately 2 minutes to complete, accommodating busy professional schedules.
BridgePoint Connects With the Patient or Family
BridgePoint will contact the patient or family to discuss their situation, explain the available services, and determine whether a consultation is appropriate.
Assessment & Recommendations
BridgePoint completes the appropriate consultation and provides the client with a comprehensive written report outlining findings, recommendations, and suggested next steps.
Continued Support When Needed
Some clients can move forward independently after receiving their recommendations. Others may benefit from additional consulting or project support
When appropriate, BridgePoint can remain involved and coordinate with contractors, builders, equipment providers, and other professionals.
You do not need to manage the accessibility project yourself. BridgePoint can remain involved at the level the client needs.
A Referral is Simply an Introduction
Referring someone to BridgePoint does not obligate the patient or family to schedule services.
Once a referral is received, BridgePoint will contact the patient or family, explain the available options, and allow them to decide whether consulting services are appropriate for their situation.
Our goal is to make the process simple for both the referring professional and the individual receiving services.
Secure Referral Process
Protecting patient information in accordance with HIPAA laws is important to BridgePoint.
Referral information and protected health information (PHI) are submitted through Hushmail’s encrypted platform. Information is handled with appropriate privacy and security safeguards, and access to client information is limited to what is necessary to coordinate services.
